SINGLE-LUMEN PICC PLACEMENT LOCATION: Outpatient, Hospital PATIENT: Troy Larson INTERVENTIONAL RADIOLOGIST: Monica J. Hamilton, M.D. EXAMINATION OF: Single lumen PICC line placement CLINICAL SYMPTOMS: Need for tong-term IV access for antibiotics; decubitus ulcer CURRENT DIAGNOSIS: 40-year-old patient with chronic infection of a decubitus ulcer in the buttock area and needs long-term IV access for antibiotics. Informed consent was obtained. The right arm was prepped and draped in the usual sterile fashion. Skin and subcutaneous tissues were infiltrated with 1% lidocaine. Under ultrasound guidance, access was obtained into the right basilic vein. Over a guidewire, the needle was exchanged for a 5-French peel-away sheath. The patient did not receive conscious sedation. The patient's pulse oximeter and vital signs were monitored throughout the exam. There were no complications. The patient tolerated the procedure well and left the radiology department in stable condition. A single digital spot radiograph obtained demonstrates the distal tip of the right basilic vein PICC to be within the right atrium. Case Study Questions:1) Please make a short summary of the patient information from the above case.2) What was found during the cystoscopy?3) Please translate any medical terms, using abbreviations wherever possible and when appropriate. Preoperative Diagnosis: Carcinoma of the urinary bladder secondary to CA of the colon; anemia and hypokalemia (an abnormally low concentration of potassium ions in the blood).Postoperative Diagnosis: Same.Procedure Performed: Cystoscopy under local anesthesia.Anesthesia: Local.Comment: This 70-year-old woman previously had a colon resection and partial cystectomy for primary carcinoma of the sigmoid colon that had perforated the bladder causing a fistula. At the present time, she is known to have a recurrence of an adenocarcinoma in the upper portion of the bladder. She has been hospitalized for dehydration and anemia, and the opportunity is now taken to check the bladder. A TUR (transurethral resection) of the bladder tumor cannot be performed at this time because her serum potassium is only 3.2, making general anesthesia dangerous.Description of Procedure: The patient is brought to the cystoscopy room. The external genitalia was prepared with Septisol, sterile water, and sterile drape. Then 2% Xylocaine jelly was instilled in the urethra for topical anesthesia. A cystoscopy was then performed using a 17 French cystoscope, using a 30-degree lens. The urine is cloudy, malodorous (bad smelling), and a culture is taken of the urine. Inspection of the bladder reveals a necrotic tumor on the upper left posterior bladder wall. There were no bleeding areas noted, so no fulgurations (the destruction of tissue by means of a high-frequency electric current) were necessary. The tumor was quite large, but did not involve the lower half of the bladder or the lower ureters. It is quite likely this patient has pulmonary metastases, as she previously had a pleural effusion and is undergoing chemotherapy.

Create a 1 – minute “elevator pitch” which explains how your RN practice will change in the next 10 years. This elevator pitch will be in an audio format. You can think about the healthcare changes learned in this course such as innovative care for aging populations, increasing numbers of individuals with chronic illness, or accreditation and standards of care. You are already enrolled in a baccalaureate program, which will prepare you academically for the future. Throughout this course, you have also practiced clinical judgment, ethical decision making, management and delegation as an RN. Identify 3 – 4 additional experiences and skills you will need to be a nurse of the future.

Case Study: Analyzing Cues Over the past three months, clients in Chamberlain Hospital's intensive care unit have had a 3% increase in central line-associated bloodstream infections (CLABSI). As a result, the unit staff has developed a clinically relevant practice question: In clients with central venous catheters, what clinical practice recommendations are available to prevent central line-associated bloodstream infections (CLABSI)? The organization's leadership team has endorsed the practice question and the clinical nurse unit educator has been assigned to lead this project. A multidisciplinary team of health professionals has been recruited to investigate high infection rates in this unit. A team conference has been held. The team has critiqued the evidence and summarized the findings. As a result, the Centers for Disease Control and Prevention's Central Line Insertion Practices (CLIP) Adherence Protocols and Checklist for Prevention of Central Line-Associated Blood Stream Infection will be used as the best practice due to the strength of its evidence. Review these two websites: CDC: Central Line Insertion Practices (CLIP) Adherence Protocols CDC: Checklist for Prevention of Central Line-Associated Blood Stream Infection For each recommendation in the left column, click to specify if it demonstrates an action to increase or decrease CLABSI. Increased immunizations among health care workers will not only slow the spread of COVID-19, but will also lessen the tollthe virus is having on the health-care professionals and individuals they are attempting to assist, according to Dr. Bailey. Non vaccinated physicians, nurses, and other professionals are likely to be seeing patients and placing them at an unnecessary high risk of catching a serious disease, in violation of medical ethics. Health professionals (HCWs) are at danger of contracting serious infections that can be fatal. If you interact closely with patients or handle infectious materials, you should acquire the necessary vaccines to lower the risk of contracting or spreading vaccine-preventable diseases. Safeguard yourself, your patients, and your loved ones.Make sure you have all of the required vaccinations.

PROCEDURE: After proparacaine was instilled in the eye, it was prepped and draped in the usual sterile manner and 2 percent lidocaine with 1:200,000 epinephrine was injected into the superior aspect of the right orbit. A corneal protective shield was placed in the eye. The eye was placed in down-gaze. The upper lid was everted and the fornix examined. The herniating mass was viewed and measured at 0.75 cm in diameter. Westcott scissors were used to incise the fornix conjunctivae. The herniating mass was then clamped, excised, and cauterized. It appeared to contain mostly fat tissue, which was sent to pathology. The superior fornix was repaired using running suture of 6-0 plain gut. Bacitracin ointment was applied to the eye followed by an eye pad. The patient tolerated the procedure well and left the OR in good condition. Read the following operative report and assign the appropriate CPT procedure code(s). Preoperative Diagnosis: Right groin lymphadenopathy Postoperative Diagnosis: Right groin lymphadenopathy Procedure: Excision of right groin lymph node Indications: Patient is an 80-year-old male with a history of lymphoma that presented to his oncologist with large right groin lymph nodes that lit up on PET scan. Intraoperative findings: Extremely enlarged lymph node. I excised about one-third of this node. Procedure: After informed consent was obtained, the patient was taken to the operating room and placed in supine position. General anesthesia was administered. The patient’s right groin was prepped and draped in usual sterile fashion. Preoperative antibiotics were given and a timeout was performed. An incision was made over the palpable lymphadenopathy in the right groin. Electrocautery was used to dissect through subcutaneous tissue, and a very large firm lymph node was identified. I initially tried to dissect around it completely; however, the node was very large and extended much deeper than initially thought. The node was grasped with Babcock and a large portion of the node was excised with electrocautery. This was approximately one-third of the lymph node. Once it was completely removed, hemostasis was obtained with electrocautery. The small lymphatics were clipped. The incision was then closed with a deep layer of 3-0 Vicryl sutures followed by a 4-0 Monocryl subcuticular stitch. Local anesthetic was injected around the incision site. Dermabond was used as a dressing. All instrument, needle, and sponge counts were reported as correct at the end of the procedure. There were no immediate complications. The patient was awakened, extubated, and transferred to the recovery room in stable condition.
