One meaningful experience for me during the oxygenation moderate fidelity simulation was caring for a mannequin representing a 74-year-old client with community-acquired pneumonia. The client presented with a productive cough and fatigue. The assessment findings included a respiratory rate of 26 breaths per minute, heart rate of 102 beats per minute, temperature of 38.4°C, oxygen saturation (SpO₂) of 88% on room air, and crackles in the right lower lobe. The provider’s order was to administer oxygen through a nasal cannula at 2–4 L/min to maintain an SpO₂ above 92%. I initially administered oxygen at 3 L/min because this was within the prescribed range. After allowing time for the intervention, I reassessed the client’s oxygen saturation and found that the target had not been reached. I then increased the oxygen to 4 L/min, which remained within the provider’s order. On reassessment, the client’s SpO₂ improved to 92%. This experience was meaningful because it showed me that providing an intervention is only one part of nursing care; I also need to reassess the client and determine whether the intervention has achieved the expected outcome. can you paraphrase this for me

code class="asciimath">Research glossary of terms Primary healthcare Primary healthcare (PHC) refers to an essential level of the health system that delivers comprehensive, person-centred services including health promotion, disease prevention, treatment, rehabilitation, and palliative care. It is designed to be equitable and accessible, particularly for vulnerable populations, and is widely recognised as the foundation for achieving universal health coverage and improved population health outcomes (WHO, 2019; WHO, 2023). Access to healthcare Access to healthcare describes the extent to which individuals are able to obtain appropriate health services when they need them. It is influenced by both health system factors-such as availability, affordability, accessibility, acceptability, and appropriateness-and individual abilities, including recognising health needs, seeking care, reaching services, paying for care, and engaging effectively with providers (Levesque et al., 2013; Cu et al., 2021). Older persons (aged 65 years and above) Older persons are commonly defined in this study as individuals aged 65 years and older in the South African context. This group often presents with increased healthcare demands due to ageing-related changes, chronic conditions, and functional decline. Healthy ageing frameworks emphasise maintaining functional ability, independence, and overall well-being in later life (WHO, 2019; Beard et al., 2016; Thiyagarajan et al., 2022). Barriers to healthcare access Barriers to healthcare access refer to any factors that prevent or delay individuals from obtaining timely and appropriate health services. These may include socioeconomic inequality, distance to health facilities, weak health system performance, and limited resources. In South Africa, such barriers are also shaped by historical inequalities and structural challenges within the health system (Coovadia et al., 2009; Ataguba & McIntyre, 2018; Cabañero-Garcia et al., 2005). Strategies for enhancing older adults’ access to PHC These strategies refer to interventions aimed at improving equitable access to primary healthcare services for older populations. They include strengthening community-based outreach through community health workers, improving responsiveness of clinics, integrating chronic disease management, enhancing transport and referral systems, and promoting integrated, person-centred care in line with universal health coverage goals (WHO, 2023; Al Shamsi et al., 2020). Health system A health system consists of all organisations, people, and actions whose primary purpose is to promote, restore, or maintain health. It includes key components such as service delivery, governance structures, financing mechanisms, health workforce, information systems, medicines, and infrastructure (WHO, 2023). Universal health coverage Universal health coverage (UHC) refers to a situation where all people can access the health services they need-ranging from promotion and prevention to t

Kona University has hired a consultant to conduct a needs assessment on the student body for a new health promotion program. Because there are few secondary data on the Kona student population, the consultant uses national data on college students overall. A public forum is proposed where students will be invited to attend the meeting on campus. To obtain data from key informants, the consultant has interviewed faculty, student advisors and administrators. As a public health student, how would you improve upon this approach? What are the limitations of the strategy used by the consultant? What additional health needs might be specific to the Kona student population that would be missed? Draft a brief letter to the consultant explaining how you as a student at Kona University trained in public health promotion would assess the health needs of students at Kona University including both primary and secondary data sources.

- Health information systems have transformed healthcare delivery
Health information systems have transformed healthcare delivery. They have changed how patient data is collected, stored, shared, and used. From early paper-based records to today’s complex EHR, interoperability platforms, and data driven decision support tools, these systems have significantly influenced patient care. Not only have they have improved access to information, care coordination, and population health management, but they’ve also introduced new difficulties related to equity, data accuracy, privacy and security. While technology has significantly changed, the need to remain ethical and compliant with legal requirements has not. The shift to digital healthcare has highlighted issues such as patient confidentiality and data misuse. Organizations must use access controls, auditing systems, and cybersecurity safeguards to address these challenges. In the future, they must continue to evolve to protect patients’ privacy and ensure administration has an understanding of important information systems to effectively lead in an evolving digital environment. Health care information systems have impacted the past, the present, and will impact the future. In the past, the reliance was on paper charts which relied on memory, limited the ability to share between providers, increased the risk of lost and damaged records, and made it more challenging to track disease outbreaks and population health trends. With the emergence of computerized health records in the 1950’s and 1960’s, experimentation with digital systems began as a way to collect, store and use patient information. At the forefront of this change, was the Mayo Clinic in Minnesota. They were one of the first large health systems to implement EHRs (Pumphrey, 2024). Dr Larry Weed created the Problem-Oriented Medical Record, which was a digital version of a medical record. It emphasized data collection, accuracy, clarity, thoroughness, and organization (Aronson, 2019). In the 1990s and 2000s, EHR gained substantial momentum. This was helped immensely by the passage of Health Information Technology for Economic and Clinical Health Act (HITECH Act) in 2009. This act incentivized health care providers and organizations to adopt EHR systems (Blumenthal & Tavenner, 2010). Today, health information systems have continued to evolve and now includes EHR’s, health information exchanges, telehealth platforms, and clinical decision support systems. These technologies have improved patient care through using EHR’s which allows providers to access patient information more quickly, as well as reducing medical errors, improving diagnoses, and supporting better treatment decisions. Health information systems have improved efficiency and communication, care coordination, and enhanced patient engagement. They also have utilized the analysis of health data to monitor disease outbreaks, track health trends, and plan public health interventions. The expanded access through telehealth has been particularly bene

- In Flack v
In Flack v. Wisconsin Department of Health Services, 328 F. Supp. 3d 931 (W.D. Wisc. 2018), a federal judge issued a preliminary injunction against the enforcement of a state regulation that excluded from coverage under Medicaid certain “[drugs, including hormone therapy, associated with transexual surgery or medically unnecessary alternation of sexual anatomy or characteristics,” as well as “[t]ransexual surgery.” On what basis did the court determine that the plaintiffs in the case had a reasonable likelihood of success such that the regulation should not be enforced during the pendency of the case? Group of answer choices (A) The plaintiffs’ evidence supported a determination that, without the surgeries requested to address their gender dysphoria, they both faced an imminent risk of suicide or other self-harm. (B) The state regulation violated Title IX and the Affordable Care Act denied the plaintiffs medically necessary surgical procedures based on their natal sex, since, under the regulation, a natal female in need of vaginal construction surgery would be approved by the state for Medicaid coverage, while a natal male needing the same surgery would be denied it due to his sex. (C) The likelihood of irreparable harm facing the plaintiffs substantially outweighed any marginal impacts on the stated concerns of the Wisconsin Department of Health Services regarding public health or containment of costs. (D) Since the initial passage of Title VII, the varieties of sex discrimination prohibited under the statute had expanded under the law to include discrimination based on a person’s failure to confirm to a certain set of gender stereotypes, and the state regulation at issue created a rule governing the medical treatment of transgender people that differed from the rule governing the medical treatment of non-transgender people. (E) Both (B) and (D).

Subscribe Nursing Shortage and Practice Implications Area of Healthcare; Long-term care How if affects this area and why it is important: This issue is defined by the projection that one million registered nurses will retire by 2030, resulting in a significant loss of clinical knowledge and expertise. In the area of longterm care, which provides essential health and personal care services over extended periods, primarily for the elderly, the impact is severe because the aging population's demographics are creating increasingly complex healthcare needs. This is critical for a holistic nurse to address because the shortage leads to increased stress, compassion fatigue, and burnout, which can dehumanize care and lower retention. Hollstic nurses have a vital role here, as they can use their knowledge of resilience, self-care, and caring cultures to influence and improve the healthcare enZironment for both patients and providers. Use of Complementary, Alternative, and Integrative Health Care Area of Healthcare: Ambulatory care How it affects this area and why it is important: Currently, approximately 33% of adults in the United States utilize CAIH practices to enhance their stat of personal health and well-being. This significantly affects ambulatory care, a specialty practice focusing on episodic encounters where patients or their families are the primary caregivers in community-based settings. This trend is important because consumers now demand personalized care and

- The primary diagnosis for H
The primary diagnosis for H.G. is acute exacerbation of chronic obstructive pulmonary disease (AECOPD) likely triggered by a viral upper respiratory tract infection. This diagnosis is supported by the patient's known history of COPD, increased use of salbutamol from baseline, worsening dyspnea, cough, sputum production, and diffuse wheezing on auscultation. According to the Global Initiative for Chronic Obstructive Lung Disease (GOLD, 2025), an acute COPD exacerbation is defined as a worsening of respiratory symptoms, particularly dyspnea, cough, and sputum production, beyond normal day-to-day variation that necessitates additional therapy. Viral respiratory infections are among the most common precipitating factors for COPD exacerbations (Global Initiative for Chronic Obstructive Lung Disease [GOLD], 2025). Furthermore, the patient's oxygen saturation of 88% indicates significant hypoxemia, which is consistent with a moderate-to-severe exacerbation and warrants close monitoring and further evaluation. Community-acquired pneumonia (CAP) remains an important differential diagnosis despite the absence of fever. The patient presents with cough, sputum production, dyspnea, hypoxemia, and focal inspiratory wheezing in the right lower lung field. Older adults and individuals with COPD may present atypically without fever or systemic symptoms (Metlay et al., 2019). Therefore, a chest radiograph is clinically indicated to exclude pneumonia as a cause of worsening respiratory symptoms and oxygen desaturation. Congestive heart failure (CHF) exacerbation should also be considered because of the patient's bilateral pitting edema (+3 right leg and +2 left leg), dyspnea, and hypoxemia. COPD and heart failure frequently coexist, and distinguishing between the two can be challenging because both conditions may present with shortness of breath and reduced exercise tolerance. Bilateral lower extremity edema may indicate right-sided heart failure, cor pulmonale secondary to chronic lung disease, or fluid overload associated with CHF (Ponikowski et al., 2021). Additional investigations such as B-type natriuretic peptide (BNP), electrocardiography, and echocardiography may be warranted if symptoms persist or worsen. Acute bronchitis is another consideration given the recent upper respiratory symptoms, cough, and sputum production. However, the presence of significant hypoxemia (SpO₂ 88%), increased bronchodilator requirements, and a known history of COPD make an acute COPD exacerbation more likely than isolated bronchitis. Acute bronchitis is most commonly viral and typically does not cause substantial oxygen desaturation in otherwise stable patients (Llor & Bjerrum, 2014). Rephrase above statements and support it with evidence from up to date instead

- Digital technologies have transformed healthcare by improving access to information,...
Digital technologies have transformed healthcare by improving access to information, communication, and patient engagement. Mobile health (mHealth) interventions, telehealth services, wearable devices, and patient portals allow healthcare providers to monitor patients remotely, deliver education, and encourage self-management of chronic conditions. According to Bommakanti et al. (2020), digital health technologies can increase access to care, improve health outcomes, and reduce healthcare costs by allowing providers to reach patients outside traditional clinical settings. Despite these benefits, significant challenges remain. One major concern identified by Bommakanti et al. (2020) is the digital divide. Many healthcare interventions assume that patients own smartphones and have reliable internet access. However, older adults, individuals with lower incomes, rural populations, and some minority groups may not have access to the technology required to participate. This can unintentionally exclude vulnerable populations who may benefit most from health interventions. Other barriers include limited digital literacy, privacy concerns, language differences, and concerns about data security. To overcome these barriers, healthcare organizations should develop inclusive digital health strategies. Programs should be accessible across multiple platforms, including text messaging, telephone-based services, and web applications. Organizations should also provide technology training, multilingual resources, and technical support to improve digital literacy. Expanding broadband access and partnering with community organizations can help reduce technology disparities. Additionally, healthcare providers must prioritize data security and patient privacy to build trust and encourage participation. Overall, digital technologies offer tremendous opportunities to improve healthcare delivery, but organizations must address accessibility and equity concerns to ensure that all patients can benefit from these innovations. what can I say to this answer
